The radiograph shows interproximal overhanging material. This causes periodontal inflammation with eventual bone loss. Proper therapy would involce the following:

  1. Removal of all amalgam and soft tooth structure.
  2. Placement of composite cores.
  3. Crown preparations and fabrication of temporary restorations.
  4. Final crown preparations and reline of temporary crowns.
  5. Impressions.
  6. Try in and cementation of final porcelain/gold crowns.
